Before applying for coverage through Humana, you’ll want to make sure your doctors and hospitals participate in their provider network.

Click on the button below to visit the Humana website. Click on the Search button under Find a Doctor. For coverage type, choose Individuals and Families. Enter your zip code. Choose the Humana / Choice Care Network PPO. You can search by name, specialty, conditions or all. Then click Search.
