Planning ahead can save frustration when you need it the least!

Because of privacy laws, insurance companies will NOT discuss your claims or membership information with another person without your permission. This applies to your spouse! Parents will not be able to discuss claims or membership information for any of their children age 18 or over, even if they are on your insurance plan.
So if your spouse is in the hospital, unable to contact the insurance company, you will not be able to contact them on your spouse’s behalf unless you have been authorized.

Every insurance carrier provides a PHI (protected health information) form that you can complete and return. The person(s) you designated on the form will be noted on your file and will be able to discuss any claims or membership issues for you in the event you are unable to contact the carrier yourself.
